By completing this survey, participants will be entered for a chance to win the designated prize. To qualify, surveys must be completed in full and submitted by 11:59 PM on Wednesday, November 26, 2025. Only one entry per person is permitted.
The contest is open to residents of Northeast Ohio who are 18 years of age or older at the time of entry. Employees of Kurtz Bros., Inc., its affiliates, subsidiaries, advertising and promotion agencies, and their immediate family members (spouse, parents, siblings, and children) or members of the same household are not eligible to participate.
No purchase necessary to enter or win. Odds of winning depend on the total number of eligible entries received. The winner will be selected at random and notified via the contact information provided in the survey during the week of December 1, 2025. If the winner does not respond within five (5) business days, an alternate winner may be selected.
